Claim submission forms

Here you find all the forms to be used to open a claim and instructions to follow depending on your case:

 

REFUND CLAIM SUBMISSION FORM

If you suffered an injury fill out this form to request the refund included in the benefits of your insurance.

You have to specify the type of policy; enter the information about the person who submits the claim (Insured); detailed description of the event; select the policy's guarantee you request for.

You can submit the claim form precautionary (if you can not define the request), to observe the 10 days deadline of the policy conditions.

If you are not able to upload documents now, you'll be able to do it when the refund claim will be opened, after completing this procedure.

 

ACCIDENT REPORT SUBMISSION FORM

To report an accident causing damage to other persons and activate the third party liability coverage of your insurance follow the instructions of this form.

You have to specify the type of policy; enter the information about the person who submits the claim (Insured); information about the damaged person (Countepart); detailed description of the event.

You can submit the claim form precautionary (if you have not received an indemnification request), to observe the 10 days deadline of the policy conditions.

If you are not able to upload documents now, you'll be able to do it when the refund claim will be opened, after completing this procedure.

 

CANCELLATION CLAIM SNOWCARE LESSONS

If you can't attend the ski/snowboard lessons insured at the ski school, send a claim using this form to request the refund included in the benefits of Snowcare Lessons insurance.

You have to specify the school where you bought the lessons and the insurance; enter the information about the person who submits the claim; specify who can not attend the lesson (Insured attendee); detailed description of the event; select your policy's guarantee you request.

You have to submit the claim before the beginning of the lesson, to observe the deadline of the policy conditions.

If you are not able to upload documents now, you'll be able to do it when the refund claim will be opened, after completing this procedure.

 

How will be handled the claim submission?

The claim will be immediately registered and sent to the management service.
You will receive a confirmation via e-mail and a temporary reference number. You'll be acknowledged about the handling of the claim with a successive e-mail, giving the claim reference number.
